New (Re) hires - required work authorization documents

Here are the work authorization documents that are required at the time of (re-) hire for:

Employees (includes all Faculty, Students, Staff, Student Interns, all PostDoc titles, and any others receiving remuneration through UC Payroll except for emeritus faculty members):

  • Oath (U.S. citizens only) - if original signature was completed within the last 12 months.
  • Patent - after one (1) day break in service.
  • Form I-9 - after one (1) day break in service.
    • Additionally, based on USCIS guidelines, Section 3 of the original Form I-9 can be used if completed within three (3) years of the date the I-9 was originally completed, or if the employee has a legal name change. Foreign Nationals require a new Form I-9 with any rehire.

Visiting Scholars/Visiting Student Researchers (VS/VSR):

  • Patent - after one (1) day break in service.
  • If they receive remuneration for living or other research-related expenses, please refer to the VSPA website for process details at:  https://vspa.berkeley.edu

Contingent Workers (CWRs) - includes Affiliates but not Visiting Scholars/Visiting Student Researchers:

Volunteers: 

  • Patent
  • Workers Compensation Program Volunteer Registration Form (to be completed by the hiring department with input from the Volunteer as appropriate)

Please note: The CANRA form applies to any applicable individual ("mandated reporter") regardless of their appointment type.
